HONEST REVIEW
- The “Surgical Catch-22”: * A significant hurdle for Oculoplasty fellows at this branch is that almost all hospital patients are “paying” cases, which are reserved for consultants.
- The Scout System: To get hands-on experience, fellows must proactively “scout” for Oculoplasty cases (DCR, Ptosis, Ectropion) from the outreach camps. If a fellow identifies a charitable case, they are generally allowed to operate; however, these cases are historically difficult to find.
- Surgical Hands-on (Oculoplasty):
- Exposure: Extremely limited hands-on for standard procedures.
- Observation: Excellent for observing common lid surgeries and DCRs. The Oculoplasty OT runs every alternate day.
- Surgical Hands-on (Cataract):
- Volume: Very low for a specialty track. Fellows are typically allotted only one cataract case per month.
- Progression: SICS is given for the first 2–3 months, followed by one Phaco case per month from the 4th/5th month onwards.
- Advanced Training: No exposure to complex cataracts, topical phaco, or iris claw lenses.
- The “Amar Agarwal” Experience: * A major highlight is the opportunity to arrive at 7:00 AM to observe Dr. Amar Agarwal perform his signature surgeries, including Glued IOLs and complex anterior segment reconstructions.
- Work Culture & Duties:
- Environment: Noted for being non-toxic and relatively relaxed (9:00 AM – 5:00 PM).
- Duties: Monthly rotations for night calls, outreach camps, and evening OPD. Expect to work 1–2 Sundays per month.
- Clinical Exposure: High diagnostic exposure to complex ocular trauma and oncology, given the institute’s status as a tertiary referral hub.
PERSONAL OPINION
Agarwal’s Chennai Oculoplasty fellowship is a “Clinical & Observational Residency.” It is ideal for an ophthalmologist who wants to master the diagnostics of Oculoplasty and observe world-class anterior segment surgery (like Glued IOLs) in a premium environment. However, due to the limited hands-on opportunities, you will not graduate as an independent surgeon. This program is best suited as a “pre-fellowship” or a prestigious addition to a resume before seeking a high-volume surgical “finishing” posting elsewhere.
